CareZ

CareZ is a program that offers financial assistance to families in the United States who have children with a severe illness, physical disability or life-threatening diagnosis.

For consideration, this form MUST be completed by a social worker or certified therapist(OT, PT, SLP) who can apply on behalf of the family in need. Please give a detailed explanation of the specific need (clothing, equipment, assistive technology, medication, or any other needs not covered by insurance). The patient must have both a primary and secondary diagnosis, with only current diagnoses considered.
